On or around June 11, 2026, Valley Kidney Specialists, P.C. (“Valley Kidney”)
became aware of unauthorized activity on our network. Upon discovery, we took immediate
action to secure the network and engaged a team of third-party specialists to investigate the
incident. Our investigation identified certain files on our network that were accessed by an
unauthorized third party on June 11, 2026.
Our review of these files remains ongoing. However, we determined that they may contain
protected health information and/or sensitive personal information and are providing this notice
to potentially impacted individuals out of an abundance of caution. The type of information
contained within the affected data may include patient names, addresses, Social Security
numbers, dates of birth, dates of service/treatment, medical insurance information, medical
record number, and diagnosis.
In response to this incident, Valley Kidney has partnered with forensic specialists to remediate
and investigate. Valley Kidney is also reviewing its existing policies and procedures related to
data security, as well as implementing new security measures to help prevent similar incidents
from occurring in the future.

Although Valley Kidney has no evidence of actual or attempted fraudulent misuse of information
as a result of this incident, individuals are nonetheless encouraged to monitor their credit reports,
account statements, and explanation of benefits forms for suspicious activity and to detect errors.

themselves regarding identity theft, fraud alerts, credit freezes, and steps to protect their personal
