Welcome to Nephrology Care Management Program

A telehealth service that provides personalized sessions, in between your regular office appointments, to help manage your kidney disease.

How It Works

The Care Management Program provides personalized phone calls, emails, and a mobile application to help manage your kidney disease.

Your dedicated medical team checks in on you digitally, and provides medical, nutritional, and lifestyle advice on how to manage your condition and live a better life, all from the comfort of your home.

You can contact your care team at any time to ask questions, find resources, or get help with your condition. Spend less time in the doctor's office and more time living your life!

Open to Most Insurance Plans!

A copay of $6-$30 may apply for some patients without a supplemental insurance plan.

What is the Care Management Program?

● This program was created by our practice to help patients like you manage your chronic kidney disease, as well as any other conditions you might have like hypertension or diabetes.
● Our team works with you to keep you healthy. We make a special care plan just for your individual situation and needs, which is reviewed and then approved by your doctor.
● Each patient gets assigned a care coordinator that will check on you regularly, in between your office appointments, to monitor your symptoms and medications, and give you advice to help you feel your best.

Why would I want to enroll in it?

Joining a care management program like ours helps you stay healthier and feel better. You get your own care team who knows a lot about kidneys. They will give you extra support and care just for you. Patients who enroll in care management can avoid ER visits and hospitalizations by managing your conditions better.

Why does my doctor recommend it?

● Living with chronic kidney disease can be challenging. For many patients, this program is a good idea because it allows you to receive care and attention between visits to the office.
● During your appointments with your doctor, they will adjust your medications and offer personalized guidance on your diet and other lifestyle changes to help keep your kidneys healthy.
● Our providers recommend this program for many patients with Chronic Kidney Disease so that they can have a dedicated care coordinator to help stick to those recommendations. By checking in with you regularly, your care coordinator can help identify any issues that may occur before your next appointment and monitor your symptoms so your doctor can adjust your care plan in a timely manner.

What patients are eligible for the program?

Patients are eligible for the Nephrology Care Management Program if they have a Medicare or Medicare Advantage plan and have been diagnosed with chronic kidney disease. Unfortunately, patients with Cigna or Independence Blue Cross insurance plans are not eligible, as their plans do not currently cover this program.
