We want to make your visit as pleasant and productive as possible. Please complete the forms in the Welcome Packet and return them as soon as possible.

What to expect at your first visit.

Your visit will begin with registration at the front desk. There we'll verify emergency contact and insurance information. You will then be taken back to an exam room where you will have your blood pressure checked and medications will be reviewed. You may be asked to give a urine sample at this time. The doctor will then come in to see you. At first there will be a review of your symptoms and medical history. Then you will be examined. Finally, the doctor will discuss his findings and suggestions for further evaluation and treatment. Don't be afraid to ask questions!

Important Points:

  • Medication List - It is very important that you bring an up-to-date medication list to every appointment.

  • Allergies - Are you allergic to any medications? Please let us know. It is important to distinguish between intolerances (examples: diarrhea, upset stomach, fatigue) and true allergies (examples: rash, difficulty breathing, etc.).

  • Family History - Does anyone in your family have kidney disease? We'll be asking about it at your visit.
